Community Living and Support (individual or group) is a service intended to increase or maintain the individual’s life skills or provide supervision needed to maximize self-sufficiency, increase self-determination and participate in their community. This service also provides the opportunity for unpaid supports to receive technical assistance that will help them ensure that achieved goals are practiced and maintained. It may be provided in the home or community and includes the following areas of skill acquisition and support:
- Independent Living
- Community Living
- Monitoring a health condition
- Monitoring a nutrition or physical condition
- Incidental Supervision
- Daily Living Skills
- Community Participation
- Interpersonal Skills
Additional requirements and rules may apply and will be covered during planning and training meetings with DDR Staff.
